• Establishing a Junior Club in the Great Lakes Region
  For new Clubs who wish to get more insight into how to start a Junior Club in Great Lakes Region.
• GL Age Waiver Request Form
  To be filled out by the Club Director for any athlete they wish to request an age waiver for.
• Injury Incident Report
  To be filled out if an injury takes place during any sanctioned USAV event or practice.
• Medical Claim Form
  This needs to be filled out by the PARENT and mailed to the Insurance company when ever a claim is being made due to an injury that occurred during USAV competition.
• Team-Liability-Insurance-Request-Form
  To be filled out for NEW Practice or Tournament Sites not used the prior year. *Facility Food Waiver, available upon request from Region office with doctors note (Please email greatlakesvolleyball@glrvb.com)*
